Assessment Scoring
The assessment questionnaire and checklist contains a series of questions for evaluating each of the nine Key Quality System Elements in the Electro Scientific Industries Supplier Continuous Improvement and Certification Standard. Each question contains one or more requirements that must be thoroughly investigated by the assessor and supported by tangible evidence before an answer is determined.
References to the Supplier Continual Improvement and Certification Standard are shown as hyperlinks. For example, Scoring Guidelines links to the actual scoring guidelines in the Qualification Procedures.
If your company is registered to ISO 9001, ISO 9002 TS 16949 or QS 9000 through a Registrar Accreditation Board or equivalent accredited registrar please complete the section below.

Scoring CriteriaEach question is given a point score from 0 to 2 as indicated below. Refer to Qualification Process for more information.
0 = Does not comply (not planned)
1 = Meets some of the requirement (planned but not developed)
2 = Meets the requirement (Implemented)
N/A = Does not apply (the question will receive a point score of 2)
Less than 75% = Not qualified
75% to 84% = Qualified
85% or greater = Preferred
This assessment also contains a weighted-point scheme for each of the nine quality system elements. Follow the weighted-point formula located below each element to compute a percentage. After completing the entire assessment, the overall rating (score) is determined by the sum of the 10 percentages.
